Existing Injuries & Medication
Please complete the relevant form below to ensure we can safely and effectively meet Ofsted regulations regarding the administration of medication or reporting of existing injuries.
Existing Injury Form Use
As part of our duty of care, it is an Ofsted Regulation that each morning nursery providers must record any visible injury that a child has obtained off the nursery premises.
Should such information be necessary to pass onto us, we ask you to complete and verify any instance on an Existing Injury Form, which you can find and print here. The forms are also available to collect and complete at drop off/collection.

Medication Form Use
We understand at times you will need us to administer medication to your child temporarily.
Should this be the case for prescribed or over the counter medication, it is an Ofsted regulation that we have certain information and your written permission to do so, and this needs to be recorded on a medication form with your signature, which you can print here. The forms are also available to collect and complete at drop off/collection.
Medication must always come into the setting in its original packaging and clearly labelled if prescribed along with the form so that a member of staff can carry out the necessary checks as part of our safeguarding procedures.
On Going Medication Form
We understand that some forms of medication are ongoing such as inhalers, eczema creams and Epi pens. Any medication that we administer needs to be recorded with your signature to say that you have given us consent to do so.
With ongoing medication, the information is recorded once and all the necessary checks carried out and then each time it is administered it is recorded and your signature obtain to confirm you know we have given it. On bringing in the medication a form needs to be completed, which you can find here. The forms are also available to collect and complete at drop off/collection.

This also allows us to regularly check that medication remains in date, so that we can inform parents and carers when it needs replacing.
